Services & Fees

The details of your personalized appointment will be tailored by both you and your provider. But in the meantime, the following information is provided to give you a glimpse into what your appointment may offer.

New Patient Office Visit:

Level 2: $100. Establish care, no prescriptions needed (new or refills), no active issues needing treatment, no new orders for labs or imaging needed.
Level 3: $145. Establish care, new order for labs or imaging, new prescription orders (max 1)
Level 4: $180. Establish care, new order for labs or imaging, new prescription order (2-3), prescription refills (<5), in depth lab/imaging review.
Level 5: $225. Establish care, new order for labs and imaging, new prescription order (>3), prescription refills (>5), in depth lab/imaging review.

Established Patient Office Visit:

Level 1: $55. Nurse visit, BP check, simple injections.
Level 2: $85. Follow-up, no prescriptions needed (new or refills), no medication changes, no new orders for labs or imaging needed.
Level 3: $115. Follow-up, new order for labs or imaging, new prescription order (max 1 prescription), simple lab/imaging review.
Level 4: $150. Follow-up, new order for labs/imaging, new prescription order (2-3), prescription refills (<5), in-depth lab/imaging review.
Level 5: $200. Follow-up, new order for labs and imaging, new prescription order (>3), prescription refills (>5), in-depth lab/imaging review.

Annual Physicals:

Females: $175 (no additional office visit fee; includes pap smear and breast exam)
Males: $150 (no additional office visit fee)
Sports Physicals: $50 (no additional office visit fee)
Medicare Physicals: $110 (no additional office visit fee)

CANCELLATION/NO SHOW POLICY:

If you need to cancel your appointment, please call at least 24 hours in advance. Appointments cancelled with less than 24-hour notice or are missed without being cancelled will result in a $35 cancellation/no-show fee.
